uni-assist's document requirements for Turkey depend on your profile
Two applicants to the same university can need different documents, because uni-assist splits its Turkey requirements by the school you graduated from. The six profiles below, and the translation rule specific to Turkey, come from uni-assist's own country page.

uni-assist splits its requirements for Turkey into six profiles, and they are not independent of one another: for Anadolu Teknik, İmam Hatip and Açık Öğretim the documents are added ON TOP of the standard school list, not in place of it. The one strict Turkey-specific rule concerns translations: the stamp must carry the phrase "yeminli tercüman" in Turkish, and a stamp in English or German only is not accepted. The conditional sentences are questions to ask yourself: have you been enrolled at a university, was only your final year at the distance-learning school, was your degree completed before 2005 with a final grade out of 100.
Bottom line
The risk is not a missing document but putting an extra one in place of the standard list. [1]
uni-assist says "additionally" for Anadolu Teknik and İmam Hatip and "also" for Açık Öğretim. On those three profiles the Lise Diploması and the Yerleştirme Sonuçları stay in force, with the four-year overview added on top.
Pick your profile, tick off the whole row including inherited documents, test each conditional sentence against your own case, then check the stamp on every translation for the phrase "yeminli tercüman" in Turkish.
The extra documents do not replace the standard list
This is the costliest misreading on the page. For Anadolu Teknik and Anadolu İmam Hatip schools uni-assist says you will additionally need to submit the listed documents, and for Açık Öğretim Lisesi that you will also need to submit them. In both cases the standard school list stays in force: the Lise Diploması and the second part of the YKS/ÖSYM result, the Yerleştirme Sonuçları.
So an Anadolu Teknik graduate does not upload only the four-year overview of subjects and grades. They must submit the certificate and the Yerleştirme Sonuçları as well, with the overview added on top. The same holds for an Açık Öğretim graduate, who adds the approved subjects list too.
Two profiles sit outside that rule for a plain reason: uni-assist restates the Lise Diploması inside its YÖS and Önlisans lists itself, and asks for the Yerleştirme Sonuçları in neither. Those are standalone lists rather than additions. [1]
The Turkey-specific translation rule: the stamp must be in Turkish
uni-assist's general rule is that you upload your certificates in their original language accompanied by a translation into German or English, and that the translation comes from a person or institution authorised to translate under oath or admissible in court, or from a department of the issuing school or university authorised to issue such translations. In its own words, it does not accept non-certified translations from standard translation agencies.
For Turkey there is one rule on top. uni-assist only accepts translations from Turkey made by a sworn translator (yeminli tercüman) whose stamp includes the phrase "yeminli tercüman" in Turkish. A stamp carrying only an English or German wording is not accepted, even where the translator is genuinely sworn.
The practical consequence is to check the stamp rather than the text when you collect the translation, and to raise it when choosing the translator. Without the Turkish wording the translation may need correcting or reissuing. [2][1]
e-Devlet, E-Okul and what makes a document official
uni-assist states that the application is fully digital: you create scans of your documents, save them as PDF files and upload them to your My assist account. What it needs is scans of official certificates, and it defines an official certificate itself: one bearing a signature and stamp from the issuing institution, a reference to its automatic issuance, or a verification code.
Reading that definition against Turkish documents is our step, not uni-assist's: e-Devlet and E-Okul printouts carry a verification code. uni-assist names neither system inside that definition. Where it does name one is the country page, which asks for the university transcript preferably as issued by e-Devlet.
E-Okul appears once, in a narrow context: for the years 9, 10 and 11 transcript owed only by applicants whose final year alone was at the distance-learning school, it says an E-Okul PDF is sufficient and screenshots are not accepted. Do not generalise that sentence to the whole Açık Öğretim overview, because the source does not. [3][1]
Degrees awarded before 2005, and how grades are read
Applying with a degree adds two rules. The first is about documents and carries two conditions at once: if you completed your degree before 2005 with a final grade out of a maximum of 100, uni-assist also asks for the examination regulations as published in the official gazette. Do not go looking for that document on the date alone, since both conditions apply together.
The second is about grades: when converting, uni-assist does not take into account the minimum pass mark for individual courses or semesters. Which university department issues the admission-basis letter, and in what format?

The page does name the source: the letter confirming which certificate formed the basis for your admission comes from your university. What it does not say is which department inside that university issues it, what language it should be in, or which format is accepted. For the confirmation of the minimum YÖS score, no source is named at all. [1]
What we tell applicants
In practice these letters come from the student affairs office, but that is our practical suggestion rather than uni-assist's rule. Ask for the certificate basis and the minimum score for that year as separate statements. If language or format is unclear, ask uni-assist rather than guessing.
Do my Turkish documents need a notary-certified copy?

The only certification rule on the Turkey page concerns translations: the stamp must carry "yeminli tercüman" in Turkish. The page says nothing about the original document needing a notary-certified copy. [1]

Do not read that silence as a firm no. The published, strict rule is the translation rule; for copy certification the binding list is the one your chosen university publishes. Keep the visa stage separate as well: its document and certification rules are their own, so check the current official list for the visa category you are applying under with the representation handling it.
Both lines describe a gap on the date shown, not a permanent state. If uni-assist publishes either point, this is the first block on the page that should change.

Frequently asked questions
As an Anadolu Teknik or İmam Hatip graduate, do I skip the certificate and the YKS result?
No. uni-assist asks for the four-year overview of subjects and grades additionally for those schools; the overview does not replace the standard list. The Lise Diploması and the YKS/ÖSYM Yerleştirme Sonuçları stay in force.
Can I use any translation agency?
